Case Study: Reversing Type 2 Diabetes Through Lifestyle Change.
Patient Profile
Name: John Topo
Age: 38 years
Occupation: Mine Worker, Mining Company
Work History: 20 years in mining life
Marital Status: Married, 4 children
Diet: Company Mess food during roster
Initial Presentation - Clinic, 2017.
John came to the mine clinic during his break. He had been feeling unwell for months but kept working because “the job must continue.”
Subjective Complaints:
- Blurry eyesight, especially when reading safety signs
- Impotence - "I can’t have sex with my wife"
- General fatigue after shifts
Objective Findings:
- Blood Pressure: 140/90 mmHg - Stage 1 Hypertension
- Blood Sugar: Averaged >10.00 mmol/L over 5 checks. Highest reading 14.0 mmol/L
- Weight: 90kg
- *BMI: Overweight category
Assessment: Uncontrolled Type 2 Diabetes Mellitus + Hypertension. High risk due to mining lifestyle, shift work, and mess food high in sugar, white rice, processed drinks, and fried food.
Intervention & Advice Given.
We sat down for 2 hours of counseling. The goal was not medicine alone, but lifestyle change he could do on site and at home.
1. Diet:
- Cut down sugar-based food. No processed juice drinks, soft drinks, or sweet tea. Live on water only.
-Eat more organic fruits, garden foods, and vegetables. Use the local market instead of the shop.
- Shopping with a list to control sugar-buying behavior. “Don’t carry too much money to the shop.”
- Eat more nuts as snacks instead of biscuits and sweets.
- Reduce white rice, bread, and fried mess food. Choose grilled fish, greens, and tubers when possible.
2. Exercise:
-Power walking for 1 hour daily within his street when at home.
- Use physical work on site: cutting, breaking ground, hauling. “Let the mine work be your gym too.”
3. Monitoring:
- Check blood sugar and BP every break.
- Come to clinic with his wife for support.
Follow-Up Results - Next Break, 2020.
After 3 years of consistent effort on and off roster, John returned for review.
Results:
- Blood Sugar: 5.5 mmol/L - Normal range
- Blood Pressure: 120/80 mmHg - Normal
- Weight: Dropped to 80kg - 10kg loss
- Eyesight: Improved. No more blurry vision
- Sexual Health: “Good. I had sex with my wife over the break.”
John was smiling. His wife was with him. He said the hardest part was the first 3 months of saying “no” to sweet drinks in the mess. After that, walking and garden food became normal.
Discussion
This case from Newcrest Gold Mining 2017-2020 shows that Type 2 Diabetes can be managed and even reversed with commitment, even in a tough mining environment.
Key factors for John:
1. Awareness: Knowing his numbers - BP 140/90, Sugar >10.0 - scared him into action.
2. Simple, local solutions: Local organic market, water instead of juice, walking in the street.
3. Family support: His wife helped with shopping lists and cooking.
4. Using work to his advantage: Physical labor on site counted as exercise.
Mining life makes health hard. Mess food, long hours, and stress push blood sugar up. But John proved that with discipline, you can take back control.
Conclusion
